Betwixt & Between

What is it?

Betwixt and Between offers a unique and alternative service for people being discharged from hospital who are not quite ready to return home, sometimes referred to as 'intermediate care'.

Avalon’s Betwixt and Between project is an exciting development meeting the needs of local vulnerable people by providing family support in the community and helping to make a pathway of support to independent living,” says Mike Beckett, Director for York and District Mind, www.yorkmind.org.uk.

The service provides support within a Family Living Service environment; where people accessing the service stay with and are supported by assessed and approved Family Living Providers within the Family Living Provider's home. The role of the Family Living Provider is to work with the customer to regain their independent living skills so they are able to return home.

The project aims to promote earlier discharge from hospital and prevent re-admission, to support the wider family network and reduce social exclusion.

Who would benefit from the Betwixt and Between service?

The service can be offered to people who have a range of needs including personal care, physical needs, mental health needs and early onset dementia. Betwixt and Between also offers a service to individuals who need a period of rest and would allow carers of individuals to take a break.

What we offer:

This one-to-one tailor-made service offers customers overnight accommodation, where the customer is involved in the planning of their support. This support is generally offered up to a period of six weeks, although this could be longer if required.

While in hospital, before the service begins, the patient will have the opportunity to meet with a member of the Betwixt and Between team to discuss the support that will be provided and to plan the transfer.

Following discharge from hospital the person will be taken to the Family Living Provider's home, who will be supporting them during this period; they will be shown around the accommodation.

A meeting will be held with their social worker or nurse at the Family Living Provider’s home to develop a plan of how the person will be supported to regain their independence, often referred to as a 'Re-enablement Support Plan'. This plan may include support areas such as moving around independently, cooking a meal safely, managing personal care and medication, managing household tasks, establishing important links in the person's local community and organising the person’s return home.

We work closely with you and other professionals, such as community nurses, occupational therapists and physiotherapists, to plan your support.

How much does this service cost and who pays for this?

This depends on the situation. In some cases this service could be funded through the continuing health care fund, a fund available for up to six weeks for individuals requiring intermediate care, which is of no cost to the individual. This would be organised between health care and The Family Living Service.

In other cases social care funds would be used to pay for the service. This would be accessed by a social worker and allocated during a stay in hospital. Individuals may be expected to make a contribution to the cost and this would be calculated by their social worker.

Some people using the service may have been allocated a specific amount of money to buy social care services known as an 'individual budget' based upon their individual support needs. This enables the individual to purchase the service directly.

The service can also be funded privately.

The initial set up fee is a one-off charge of £70.

Thereafter the weekly cost of the service is £580.72.
